Food Questionnaire

The food questionnaire is very important to me providing the best service to you the customer. It provides me with a basic understanding of what your food likes and food dislikes are.  A personal chef service is a service that only gets better. My job is to take this knowledge of what you like coupled with any new knowledge I acquire about what foods I make that you like, and from there learn to really personalize the service. The more honest you are about what your food likes and dislikes are the better I can serve you.

Follow the link and fill out please to the best of your ability.
